Privacy Policy
Decision Point Healthcare Solutions ("DPHS", "We", or "Us") operates the Member Profile and Opus web applications (together the "Platforms"). Only authorized employee of DPHS's clients and its client's subsidiaries (together the "Organization") who are atleast 18 years of age may access and use these Platforms. These Platforms collect and store limited personally identifiable information. DPHS will not use or share your information with anyone except as described in this Privacy Policy.
Personally Identifiable Information
DPHS respects your privacy. We do not collect any personally identifiable information from users. All personally identifiable information about users is provided by the Organization to DPHS to enable us to identify users and grant them access to the Platforms. This information may include your:
- name
- email address
- employee ID
- date of hire
- job role
- other employment-related data that is relevant to your use of these Platforms
Organization does not provide DPHS with your social security number or date of birth.
We do not sell, rent, or loan any personally identifiable information regarding our users to any third party.
Data Security
Your account can be accessed in one of two ways:
- Username & passphrase - When you access your account using this method, you are required to set a passphrase through the platforms. The passphrase is stored in an irreversible cryptographic hash that enables DPHS to validate the passphrase but not toread it. For your own security, we recommend you set a unique passphrase that has not been used on any other web site or service.
- Single Sign On - When using the Single Sign On capability provided by the Platforms, you will use the credentials issued by the Organization to gain access to the Platforms. DPHS creates a secure encrypted connection between the Platforms and the Organization so that your access credentials and supplementary metadata cannot be intercepted.
Information that is passed between your browser and the Platforms is encrypted by HTTPS. This creates a protected connection between your device and the DPHS server to ensure confidentiality of transmitted information.
Cookies
A cookie is a small file saved by your Web browser to assign a unique identification to your computer. We use cookies to keep your log-in session active and to track your activity on the Platforms. We do not use cookies to retrieve personal information about you from your computer. If you disable cookies on your computer, you will be unable to log into and use the Platforms.
We do not use any cookies or any other technology that can track your activity on other websites or Platforms or correlate your activity on these Platforms with any activity on any other website or service.
Logging
Whenever you visit the Platforms, we collect information that your browser sends to us that is called Log Data. This Log Data may include information such as your computer IP address, browser version, operating system, Platform URLs that you visit, searches and other actions you perform, the time and date of your visit, healthcare records viewed, and other statistics. Log Data includes a unique random identifier that is assigned to you but does not include any of your personally identifiable data. Log Data is used to monitor the performance and security of the Platforms.
Log Data is disclosed to the following:
- All Log Data is available to the Organization.
- Some Log Data is disclosed to third-party traffic analytics services.
- Log Data may be disclosed to other parties as permitted by law in the investigation of a possible security incident or breach of patient data under HIPAA.
HIPAA
You are required to follow the HIPAA policies and procedures required by DPHS and the Organization to safeguard access to your account and protect the privacy, security, and integrity of the Protected Healthcare Information ("PHI") that is accessible by the Platforms.
California Do-Not-Track Disclosure
DPHS is required by HIPAA to track all user activity and does not change tracking in response to any Do-Not-Track browser requests.
Thrid-Party Links
The Platforms may contain links to other sites. If you click on a third-party link, you will be directed to that site. Note that these external sites are not operated by us. We have no control over, and assume no responsibility for the content, privacy policies, or practices of any third-party sites or services.
Updates
We may update the Privacy Policy from time to time. Thus, we advise you to review this page periodically for any changes. We will notify you of any changes by posting the new Privacy Policy on this page. These changes are effective immediately, after they are posted on this page.
